Changes at Symphony Healthcare:

Insurance: As of October 1, 2024, Symphony Healthcare will be out of network with commercial insurance plans, but will remain in network with Traditional Medicare Part B (First Coast – or the “red, white, and blue Medicare card”)

Multi-Dimensional /Functional Medicine Visits – for those of you who want to receive a comprehensive, functional / multi-dimensional approach to your health issues, we will provide primary care services but will be focusing on helping patients with Thyroid, Hormone, Auto-Immune and Gut Microbiome issues.

By not having to comply with insurance regulations, I can provide the benefit of more detailed visits, functional root-cause analysis of labs & testing, personalized holistic treatment plans, messaging between visits, telemedicine visits, along with discounts on in-office supplements, injections, and self-pay rates for labs. Our Functional Medicine patient panel will be limited, to provide this personalized care that truly is my passion. This is how I can help you truly get healthy. If you choose to leave the practice, we will transfer medical records electronically to a new primary care medical provider of your choice at no fee, with a signed consent form. Only Medicare insured patients will have insurance claims filed. 

New Hormone & Weight Management Concierge Package Programs – will provide additional information on these new programs soon

New Package Programs for a Comprehensive Functional Medical approach  – will provide additional information on these new 3 and 6 month programs soon

Individual Self-Pay Visits –

  • For new patients the first visit fee will be $250.
  • Standard Follow up visits will be $100 per visit up to 30minutes.  $150 up to 45minutes.  $200 up to 60minutes.  $100 for an additional time up to 60 minutes – Visits will be scheduled as needed for your unique health issues.
  • A “Super Bill” can be created at the end of the visit if requested for those with out of network insurance benefits – the patient can send it in for reimbursement.
  • Wholesale pricing is available for self-pay labs.

Medication Refills – To be done at the time of your visit to confirm the correct dose and use.

On-Line Scheduling – We will be offering on-line self-scheduling soon.

General Questions – Leave a detailed message on our phone line at 352-629-5939 (Please include: spelling of your name, date of birth, call back number, and your detailed message) or send a message via your patient portal on PatientFusion or via e-mail to Scheduling@SHCOcala.com. Questions will be answered within 24-48 hours.  Medications are only refilled at time of visits.

“Optimantra” is no longer functional. 

“PatientFusion” will be your new portal  – Please sign up for this secure, HIPAA-compliant portal –for email & text messaging, document-sharing & Telemedicine visits. You have access to your patient information profile to view and update as needed. You will also be able to view lab reports when completed.
