New Patient Paperwork! If you are a new patient, please complete your Patient Portal in advance to help ensure your appointment runs smoothly and on time. To go to your Patient Portal, Click Here! Please upload your driver’s license and a list of any medications or supplements you are currently taking or any other reports you may find helpful if they have been done in the last 3-6 months. We also request that you be ready for your appointment 10 minutes before your appointment time to allow us to properly process your telemed link or prepare for your office visit.
Request for Medical Records. If you are requesting Symphony Healthcare Inc. or Synergy Health Consulting to either release or obtain medical records on your behalf, we require this form to be completed and submitted to our office to process your request. You are also able to fax this to our office at 352-629-7833.
