Insurance & Financial Information

INSURANCE:

We have made prior arrangements with many insurers and health plans. We will file claims with those

plans with whom we have an agreement with. We will collect any required co-payment, co-insurance, or

deductible at time of service. In the event your health plan determines services to be “not covered,” you

will be responsible for the complete charge.

We are currently in network with:

We accept limited HMO policies through Florida Blue and United Healthcare. Please Contact Us for more information and to verify if we are in network. We do not file secondary insurance unless there is an automatic crossover from Medicare. If you are interested in becoming a patient and you are unsure if we are in network with your plan, please either contact your plan directly or our office for verification. You also have the ability to be seen out of network and submit the claim to your insurance company directly. Furthermore, we have extremely attractive and affordable self-pay rates for both office visits and lab tests. Please contact us for more information.
